I went to file 11B today and it was complete nightmare. I got up to the window to Swear my Notice of Application. Girl asks me for the Affidavit. I tell her that there no need for affidavit right now, because I haven't faxed the Application yet. She tells me: go serve the Prosecutor. Can anyone proof read my 11B application. I included two charges in one application, because I got them at the same time and my trials were at the same time for both tickets.

Also, I got two tickets at the same time and I had one courtdate for both charges.
Do I have to do all the paperwork for each ticket?

UPDATE:
Back in August 2011, I got a call to come pick up the disclosures. Never got around to do that.
Should I pick them up or forget it since I am filing 11B?

I got a question about Step 5: Tell clerk that you would like your 4 copies of the Notice of Application sworn by a commissioner of oaths. (see screenshot below)

This part is missing from the Notice of Application for Stay of Proceedings template I downloaded. Is commissioner of oaths will insert it or I have to include into tab2 myself?

Here is the timeline so far: July 2010 - Charged Aug 2010 - First attendance (meeting with the prosecutor) July 2011 - Courtdate Jan 2012 - New courtdate appointed by the prosecutor because disclosure wa not provided. Hi,

My courtdate was moved due to prosecutor not providing me with a disclosure. Because of this mistake, my court is now 17 months since I got charged. My next step would filing 11B motion.
I am a little confused on which forms to use. Can I use Affidavit form and Form 4F? I attached the copies of the forms, please look it over please to see if I got the right ones.

Here is another questions, my court is at 3pm and I want to try to change it to 8am. If I am lucky and manage to do it, is it going to affect my ability to file 11b motion? Since, technically I moved the court date.
