I didn't use form 4F. I followed the process described in this topic: http://www.ontariohighwaytrafficact.com/topic2808.html You need to swear that facts in your application are correct. Then serve other parties starting from Attorneys. After faxing to Attorneys, go to Comissioner of Oath again and ...

You can call to 860 William Street, Cobourg and ask them where to send documents to court and prosecutor. I am trying to estimate which options I have now. As far as I understand, my opportunity just to pay the fine which is on the ticket ($95) without visiting court has gone since I applied for a trial. What is smth? = Smith? smth = something Courts plan there time based on pleas (5min) and trials (30min +). If they know then have 10 pleas that will be 1 hour to get done moving at a fast pace. I received a notice of trial (HOV lane) which was set 12 month and 1 week after a tickets date . I sent disclosure request 7 weeks before the trial and didnt get an answer. After 3 weeks, I sent another disclosure request and didnt get any answer again. Now my trial day is coming in a couple of days...
