Can Anyone Help Me Out On How To Read A Police Report?

Hello all,
Somebody rear ended me last week pretty badly.
My car is a write off and the officer who attended the scene emailed me his report.
I can see that the other driver was charged with careless driving but I can't seem to figure the rest out.
The report has numbers along the side of the page and the officer put different coloured circles corresponding to myself and the other driver.
It seems like I would need a reference page to see what each number refers to?
I would assume it's your typical motor vehicle accident report.
The form says: SR-LD-401 2012/05
