Form 4f Question (4a? 4b?)

Hi All,

I am in the process of requesting a "stay" because they are not providing all the necessary disclosure that I requested.

I need to fill out the 4F form and close to the top of the form the following can be found "(General heading)"

It looks like whenever the words "(General heading)" is seen, a 4A (GENERAL HEADING OF DOCUMENTS — ACTIONS) or 4B (GENERAL HEADING OF DOCUMENTS — APPLICATIONS) form needs to be filled out as well.

I've searched many sites and forums and all the examples provided do not talk about 4A/4B forms and I'm having a hard time finding examples.

So the question is: Do I absolutely need to fill them out? According to http://www.ontariocourtforms.on.ca/english/civil we have to. Furthermore, we have to follow "the rules of the court"...

Any help is greatly appreciated...

What were you charged with? What did you request? What didn't you get?

In a vast majority of the cases, the remedy for lack of disclosure is an adjournment. If you believe that the prosecution is not providing you with something that you feel is necessary for your defence, then you should be prepared at your next court appearance to argue that fact in front of the JP and then they will make a decision.
