Page 1. An ICON print out of my info, what's the significance of this?

Page 2. My Letter requesting for full copy of police notes, copy of both sides of ticket, witness will say , witness statements, any statements made by defendant, copy of original notes of such statements, make and model and serial # of radar/laser unit, any hand written information to be typed
